• Support CCC Activities: Collaborate with clinical staff to ensure smooth operation of CCC activities
• Integrate healthcare services in CCC department focusing on Mother and child health, CWC (Child Welfare Clinics), family planning, cancer screening, and other clinics
• Patient Management and Follow-Up: Organize follow-ups for patients who have defaulted on treatment
• Provide adherence counseling and manage patients with high viremia
• Offer skilled nursing procedures, including dressing changes and emergency response
• Educational and Community Engagement: Lead educational sessions at CCC
• Stay updated with best practices and guidelines related to patient care and clinic management
• Support community outreach and mobilization efforts for care and support
• Assist in forming and managing patient support groups
• Patient Counseling and Confidentiality: Provide pre-ART preparation, adherence counseling, and family education
• Ensure patient confidentiality is maintained
• Clinical Procedures: Administer medication, perform phlebotomy, random blood sugar tests, and initiate IV infusions as required
• Exercise appropriate judgment and decision-making in patient care
• Administrative and Supervisory Responsibilities: Facility Management: Oversee clinic activities, including petty cash handling, Routine Activity budget and accountability, report preparation, drug orders and stock management
• Serve as facility in charge, managing assets and inventory
• Supervision: Supervise and support ART Aides, CHPs, and other clinical staff
• Taking on team leader role, assisting wit provision and supervision of patient care
• Reporting and Advocacy: Report significant changes in patient status to relevant medical providers
• Act as advocate for organization (AHF) at site level
• Other Duties: Fulfill additional duties as assigned by supervisor.

Patient Care Duties

• Identify Care Requirements: o Build rapport with patients and their families to understand care needs.
• Provide Emotional and Supportive Care: o Offer emotional, psychological, and spiritual support to patients and their families.
• Promote Independence: o Set patient care goals, educate patients and families about conditions, medications, and self-care, and address their questions. • Document Care Services: o Record patient care activities and updates accurately in patient and department records.
• Maintain Confidentiality: o Ensure patient information is kept confidential and operations are protected.
• Foster Team Collaboration: o Communicate effectively with healthcare teams, respond to requests, build rapport, and participate in team problem-solving. II. Administrative Duties
• Ensure Quality of Care: o Adhere to therapeutic standards, measure health outcomes against goals, and make or recommend adjustments as needed.
• Resolve Patient Problems: o Utilize multi-disciplinary team strategies to address patient needs and issues.
• Maintain Continuity: o Document and communicate actions, irregularities, and continuing needs among nursing teams.
• Manage Supplies Inventory: o Monitor stock levels, anticipate supply needs, place and expedite orders, and verify receipt of supplies.
• Enhance Professional Knowledge:
• Attend workshops, review publications, network with professionals, participate in societies, and ensure nursing council license renewal every three years.
• Oversee Equipment Operation: o Complete maintenance, troubleshoot malfunctions, call for repairs, maintain equipment inventories, and evaluate new equipment.
• Collaborate with Healthcare Teams: o Communicate, respond to requests, build rapport, and engage in problem-solving with team members. III. Safety and Environmental Duties
• Maintain a Safe Working Environment: o Follow procedures and regulations to keep the environment safe and clean, and seek assistance from support personnel as needed.
• Adhere to Infection-Control Policies: o Follow infection control policies, medication administration and storage procedures, and regulations for controlled substances.
• Contribute to Team Effort: o Accomplish related tasks and contribute to the team’s overall goals as needed.

• Coordinate Nursing Services: Oversee all aspects of nursing care, including maternity services, child welfare clinics, immunizations, family planning, antenatal and postnatal care
• Ensure comprehensive prevention strategies for mother-to-child transmission of HIV
• Foster Professional Relationships: Build and maintain effective communication and collaboration among different health professionals within facility
• Collaborate with Health Project Partners: Work with organizations like Achap, Afya PLUS and AMREF to enhance client service delivery
• Lead AYSRH Initiatives: Act as primary contact for Adolescent and Youth Sexual Reproductive Health (AYSRH) and coordinate all related activities.

• Clerking of Patients: This involves the initial assessment and documentation of patient information, including their medical history, current symptoms, and any other relevant details
• Prescriptions and Administration of Drugs: Prescribing medications based on the patient's needs and administering these drugs, ensuring the correct dosages and monitoring for any adverse effects
• Minor Surgical Procedures: Performing or assisting with minor surgeries or procedures, which could include things like suturing wounds or removing small growths
• Assessing, Planning, and Prioritizing Patient's Needs: Evaluating the patient's condition, developing a care plan, and prioritizing tasks to address the most urgent needs first
• Implementing Appropriately: Carrying out the planned care and interventions according to the established protocols and patient needs
• Referring Patients: When necessary to specialists or other healthcare providers for additional evaluation or treatment
• Offering Long-Term and Short-Term Family Planning Services: Providing guidance and services related to family planning, which include contraceptive options, fertility counseling, and planning for future pregnancies
• HIV Counseling & Testing and Appropriate Referral: Offering counseling on HIV prevention, testing for HIV, and referring patients to specialized care or support services if needed
• Documentation and Record Keeping: Maintaining accurate and detailed records of patient interactions, treatments, and outcomes to ensure continuity of care and for legal and administrative purposes.
