Accomplishments

  • As the head of the Internal Medicine Department, I oversee both inpatient and outpatient services in a facility that has limited resources. Our inpatient service accommodates approximately 55-60 patients per month, while our outpatient service handles around 100 patients per month. Despite our limited resources, we strive to provide comprehensive care to promote the health and well-being of all our patients.
  • As the Chairperson of the Medical and Therapeutics Committee (MTC), I lead meetings with a team to develop and implement guidelines for the appropriate use of medications and supplies in our healthcare facility. I also oversee interventions to improve the usage of medicine.
  • I am also responsible for implementing the SQuID protocol for managing patients with mild to moderate severity diabetic ketoacidosis (DKA) outside of the ICU. This protocol involves administering fast-acting insulin analogs and ensures safe and effective DKA management while optimizing patient care in our resource-limited setting.

Contributions to Quality Improvement

  • I conducted an audit on the implementation of the 2019 European Society of Cardiology Dyslipidaemia Guidelines among post-myocardial infarction patients who were receiving treatment in a medical facility in Nairobi. The study revealed that there were gaps in adherence to the guidelines, including inadequate follow-up of lipid profiles to assess LDL-C goal attainment, and suboptimal use of non-statins to achieve recommended targets. The study recommends strategies such as widespread dissemination of the guidelines among healthcare workers to improve secondary prevention strategies in this high-risk cardiovascular risk group.
  • As a member of the editorial committee, I collaborated with the Kenya Cardiac Society and the Ministry of Health of Kenya to update the Kenya National Cardiovascular Guidelines in 2018. My contribution to the editorial committee involved a rigorous and comprehensive review of the existing guidelines, followed by the identification and incorporation of the latest research and evidence-based practices.
  • I was a member of the task force responsible for establishing a dedicated COVID-19 hospital that treated infected residents of Kiambu County during the pandemic. This involved renovating an old hospital premises to create a Severe Acute Respiratory Infection (SARI) Centre, while following strict Infection Prevention Control measures. We also zoned patients based on the severity of their illness and designed the flow of both patients and staff into clean and dirty corridors.

Contributions to Teaching and training

  • In my current role, I provide instructional sessions at the bedside and in clinical settings to medical officers, clinical officers, and international medical students who are completing external rotations at Tigoni Level IV Hospital. I am also responsible for the implementation of Continuous Medical Education (CME) training programs and regularly deliver lectures.
  • During the COVID-19 pandemic, I completed a training of trainers course focused on the diagnosis and management of COVID-19. Subsequently, I trained over 120 hospital staff members.
  • During my Masters training, i participated in conducting examination for the 5th and final year medical students. This would involve selecting patients for clinical examinations and assist in conducting the Objective Structural Clinical Examination (OSCEs)

Additional Information

  • During my medical school years, I actively participated in medical camps organized by the Christian fellowship. These camps took us to remote areas in Kenya, where we provided free medical services and medication to disadvantaged communities. This experience deeply ingrained in me the importance of accessible healthcare for all and solidified my commitment to serving those in need.
  • Five years ago, upon earning my Master of Medicine in Internal Medicine, I purposefully redirected my career trajectory toward serving the public sector. Choosing to work at a hospital on the outskirts of Nairobi, despite its resource limitations, was a mindful decision. Despite facing various challenges, the experience has been deeply fulfilling. It has afforded me the chance to make substantial contributions to improving healthcare access within underserved communities.
